Faulty engine chain/oil pump

We have just had the engine chain replaced on our car..we got it back and it had a loud whining noise when driving..mechanic took it back and said it was caused by this and then that and then something else before saying it was the oil pump..he reconditioned the oil pump which helped the noise, it wasn't as loud but still there, he knew we weren't confident but said it was safe to drive. This was done about a month ago. A couple of days ago we broke down, we were doing 70 and we heard a big clunk and bang, the oil light appeared and we immediately stopped the car. We have now been told the oil pressure relief valve got stuck and the spring inside has possibly snapped. What could have caused this? If there was a fault is that something he should have picked up when reconditioning the pump or changing the engine chain or trying to find out where the noise was coming from?
